We have created some general office guidelines that we kindly ask all patients to review and follow.
Respect
- Please treat all clinic staff with respect.
- If you are kept waiting, we appreciate your patience as we are working to provide the care you need.
- Aggressive behaviours or any form of verbal or physical abuse towards staff or others in this office will not be tolerated.
- Please bring ALL of your medications to each visit (original bottles or Blister Packs)
- If you require a refill of medications, please contact your pharmacy and ask them to send a prescription refill request to your health care provider. Your health care provider may require blood work or an appointment prior to renewing certain medications.
- Please do not wait until you are out of your medications to request a refill. Please contact your pharmacy at least 2-3 weeks prior to running out of your medications or when you fill your last refill at the pharmacy.
- For your appointment, you may be offered to see your family doctor, nurse practitioner, or another doctor that is covering for your family doctor while they are away.
- During an appointment, we can address 1 or 2 medical issues. If you have more concerns, we may have to book additional appointments.
- If you arrive late for your appointment, you may only be given the remaining time in the appointment or be asked to reschedule.
- Please call or book an appointment online to schedule your appointments.
- Please give as much notice as possible when canceling an upcoming appointment.
- If it is an urgent appointment we will try to fit you in as soon as possible. When you book a same-day urgent appointment, we will deal with your urgent health issue. You may need to book another appointment to deal with other non-urgent health issues.
- Depending on your health issue as well as external circumstances, you may either schedule a phone appointment or an in-person office visit with your healthcare provider. The health care provider may change your visit type depending upon the health issue.
- If you miss your appointment, you will be charged a $10 fee
- Many forms are not covered by OHIP, therefore you may be billed. You will be notified of the cost at the time of the request.
- All patient sections of forms are to be completed before giving the form to the health care provider to complete.
- Please allow time for the form to be filled out. The lengthier forms may require a longer time period to complete.
- If you have concerns about the fees please bring this to our attention.
